Reconcile Carrier Bills Before You Pay Them — Not After

Every invoice line matched to the election that caused it, the deduction that funds it and the carrier’s own eligibility file — so the premium for someone who left in March is a dispute in April, not a write-off in December.

Carrier bills are the largest recurring payment most HR teams approve, and the least examined. Benefits Reimagined treats each invoice as a claim to be proven: the line has to match an active election, a payroll deduction and the carrier’s eligibility record before it is approved, and the exceptions — terminated but still billed, wrong tier, missing dependent, duplicate — are worked as cases with a dollar value attached.

The Bill Nobody Reads Line by Line

“The invoice is 40 pages. We check the total against last month, approve it, and once a year a broker audit finds the people we’ve been paying for since they left.” Premium leakage is not a carrier error — it is the gap between the HR record that knows someone left and the invoice that never heard.

Every month

Paying for people who left

A termination keyed in the HRIS never reached the carrier, or reached it late; the invoice keeps the line, and the approver checks the total, not the names.

Every month

Tiers and dependents that don’t match

Family billed where employee + spouse was elected; a dependent who aged out still covered; a duplicate line for a rehire — each small, each recurring.

Every correction

Retro deductions nobody can explain

A mid-year fix creates a catch-up deduction; the employee opens a ticket; nobody can say which month, which plan or why.

4sides to every line — the invoice, the election, the payroll deduction and the carrier’s eligibility record
$0.01default tolerance; anything outside it becomes a typed exception with an owner and an amount
$7+per employee per month of premium leakage found in industry studies — an industry figure, not a BR result

One Billing Period, Four Controls

Follow one month’s invoices from intake to posting — each control is an existing module, and each links to its own page.

1

Take the invoice in and match every line four ways

Carrier invoices arrive as files or PDFs and are parsed into lines. Each line is matched to the employee’s election in Benefits Reimagined, the deduction payroll actually took, and the carrier’s own eligibility record — the four-way reconciliation, where the source must equal what is accounted for plus what is explained. Matched lines are approved under the tolerance you set; everything else becomes an exception with a reason.

2

Work the exceptions as cases with a dollar value

Terminated but still billed, tier mismatch, missing or extra dependent, rate variance, duplicate — each exception type has an owner, a resolution method and an amount. Disputes go to the carrier with the evidence attached; corrections that belong to enrollment go back to the record; follow-ups carry a 3-, 5- or 7-day clock; the approver sees the period’s net exposure, not a 40-page PDF.

Fix the deductions — and explain every retro

When a correction changes what an employee should have paid, BR ReVerge — 360-degree Payroll — computes the retroactive difference by pay period, gets the pay distribution approved in Workflow, and transmits the locked run to SAP ECP or any 3rd-party payroll (ADP · Workday · UKG · Dayforce · Paycom · SAP ECP) with the explanation the employee will see. Premiums for people not on payroll — COBRA, unpaid leave, retirees, survivors — are billed directly, with grace computed from dates and payments.

Approve in Workflow, post with the variance explained

Approvals happen only in BR Workflow. The approved amount posts by cost center and legal entity with the disputed lines held; a closed period blocks posting and a carrier mismatch blocks accept, transmit and post. The period’s variance against elections and deductions sits on the Essential Insights tiles, and the next invoice starts from a carrier file that already reflects the corrections.

What Changes When the Invoice Meets the Record

From checking the total

Matching every line

Each line proves itself against an election, a deduction and the carrier’s eligibility before it is approved.

From a broker audit once a year

A dispute every month

Terminated-but-billed lines are found in the period they occur, with the termination date attached.

From a 40-page PDF

An exception queue with amounts

Each exception has a type, an owner, a resolution method and a dollar value; the approver sees net exposure.

From retro deductions nobody can explain

ReVerge, by period, approved, with the why

The catch-up or refund is computed per pay period and the explanation goes to the employee before the paycheck does.

From premiums tracked on a sheet

Direct billing by reason

COBRA, leave, retiree and survivor premiums billed, reminders sent, terminations reviewed — never a silent flag.

From a number that doesn’t tie

A posting that does

Approved in Workflow, posted by cost center and entity; the variance is on the tiles, not in an email.

Ask What You’re Paying For and Why

The billing workspace has Athena for Admins. It reads the period’s invoices, matches, exceptions and approvals, explains any line in plain language and drafts the dispute or the correction — a person approves it in Workflow. It never claims to have posted or transmitted anything.

  • “Why is the medical invoice $38k higher than the elections?” — the 41 terminated employees still on the file, by termination date, with the dispute drafted
  • “Which exceptions are worth chasing first?” — the queue ranked by amount and follow-up clock, with the carrier’s response history
  • “Explain this employee’s retro deduction.” — the correction, the periods affected and the amount per paycheck — in words the employee can read
  • “What did we recover this quarter?” — disputed, credited and accepted-as-is, by carrier

Why is September’s medical invoice $38k over what the elections say?
Athena · from the September match41 employees on the invoice have a termination in Employee Central dated between May and August but are still on the carrier’s eligibility file. The four-way match flagged each line as ‘terminated, still billed’ — $38,412 this period, and $71,900 across the periods since their termination dates. 38 of the 41 were on the 834 termination file the carrier acknowledged; 3 were keyed after the file cut and were never sent.
